2009 KAW VALLEY FOOTBALL AND CHEERLEADING REGISTRATION FORM The Kaw Valley Football and Cheerleading Club (KVL) is now accepting registrations for youth tackle football and Cheerleading. Space will be limited. Preference will be given to players who have previously played in KVL .Whenever possible, players will be assigned to teams according to the school or school district they attend. Final Rosters will be decided by the KVL Commissioner. All coaches will be approved by the KVL Commissioner. Most games will be played on Saturdays at the Field of Dreams located in Basehor,Ks. Additional games may be played in other communities within the area. Each team will play a minimum of seven (7) league games( weather permitting). At the end of the season all teams will advance to a League Superbowl or a Bowl game. League play will begin on Sept 12,2009. The KVL will provide insurance,awards( for all players and cheerleaders), referees and a well maintained facility. Each player will provide their own football equipment (rental is available), mouthpiece, shoes, jersey, football pants w/pant pads and athletic supporter (The KVL will have a limited supply of equipment available to rent)The league will be divided into the following divisions (subject to Change) Flag (K-1) Tackle will be 2nd grade, 3rd grade, 4th grade, 5th grade, 6th grade. The KVL League Commissioner will rule on all issues not clearly stated in league rules. All players must have a current athletic physical (completed in 09), proof of 2009 grade and a copy of their birth certificate turned into the League Commissioner prior to the first practice. NO EXCEPTIONS! I understand medical insurance is not provided with KVL programs. I release the KVL from any and all liability whatsoever resulting from participation in KVL activities. I hereby waive and release the NYSSO and the KVL, it's staff, agents, sponsors, and/or coaches from any and all liability that may occur from accident, injury or illness sustained by my son/daughter during participation in these activities. I understand that no refunds will be applied within two weeks of the beginning date of a program. I understand behavior resulting in removal from a program does not constitute refund criteria. I understand that refunds, when applied, will have a $20.00 administrative fee accessed. I understand that if equipment is issued in conjunction with any program, failure to return said equipment within 2 weeks of the end of the program will result in legal action. I understand that any photographs, medals, awards trophies, etc., associated with programs may be held for 30 days after the end on the activities at which time, if not claimed, will be disposed of. I understand returned checks will be accessed a $20 processing fee. I understand that photographs of all NYSSO and, KVL activities and activities conducted by leased tenants will be taken and may be used for brochures, promotions and advertising without permission.
